Home and Community Care - Health Care Worker 

Casual or Part Time

The Home & Community Care Health Care Worker (HACC PCW) is responsible to support older people, people with a disability and their carer to maintain their dignity, privacy, freedom and independence. The worker is expected to work within the Active Service Model (ASM) of care focusing on promoting capacity building and restorative care with a person-centred approach to care, promoting wellness and active participation in goal setting and decisions about care. The HCW works as part of a team that includes the district nurses, allied health and community health workers. The worker is expected to make routine decisions in the course of their work, but to refer to more important decisions to the Home Care Coordinators. The worker is expected to know how to act in an emergency situation in accordance with Corryong Health safety and security policy and to understand which decisions they can make and which need to be referred.

Reporting

This position reports to the Home Care Coordinator.

Primary Roles and Responsibilities

  • Provide practical and personal support to eligible and frail aged residents, residents with disabilities and families to maintain a quality of life and/or prevent early institutionalisation.

  • Provide respite for usual full time carers of physically and intellectually disabled children and adults (specific).

  • Household domestic cleaning tasks.

  • Perform tasks associated with personal assistance, such as assisting with showering, assisting client with shopping, banking, paying accounts, writing letters, assisting with cooking meals, and assisting clients with other duties as requested, including supporting people with self-medication in response to assessed need by HACC Coordinator or District Nurse.

  • Provide social and emotional support and companionship to the client or the carer of the client

 

Requirements

  • Evidence of an approved course in Certificate III Aged Care through a recognised training organisation.

  • Evidence of an approved or willingness to work towards Medication Unit including Recognise Healthy Body Systems in a Health care context and Assist Client with Medications.

  • Demonstrated understanding of the needs of frail older people, their carers and people with a disability and their carers.

  • Demonstrated a capacity and ability to work self-directed with minimal supervision.

  • Experience in performance of a range of household duties.

Registered Nurse and Midwife

Tracey Damm

I’m an ANUM here at Corryong Health and have been working here for over 20 years. My role is very varied and rewarding. Working and living in a small community means we usually do something different every day.
I run a small Day Procedure Unit which we mostly perform Endoscopy with a visiting Surgeon from Albury Wodonga and a visiting Anesthetist from Mt Beauty.
We have a 2 chair Dialysis Unit with 2 permanent patients.
I also run an Antenatal Education program and do the Domiciliary Visits for Albury Wodonga Health when Mum and Babe first come home from hospital.
Rural Nursing is a very rewarding career with a huge variety of clients coming to the facility. Corryong Health is a very important part of the community.
