The Community Mental Health Clinician is a member of a multi-disciplinary community health team
providing an integrated mental health service and support for individuals and families. The Community
Mental Health Clinician works with local and regional mental health service providers to develop pathways
and shared care planning to support people in crisis. The Community Mental Health Clinician provides
psychiatric, social and physical assessment of patients/consumers and delivers clinical treatment and care to
resolve or ameliorate emotional distress within the community.

Job Description


This position reports to Mental Health Manager.

Primary Roles and Responsibilities

  • Work with local and regional mental health service providers as part of an integrated mental health service ensuring the provision of rehabilitation, recovery, support and discharge planning consistent with consumer and family/carer(s) needs.

  • Provide assessments and intensive outreach treatment and support for consumers experiencing mental disorder and illness.

  • In collaboration with each consumer, carers, families and other care providers, develop an individual care plan consistent with Victorian MHA 2014, based on a comprehensive assessment of the consumer's needs; contains specific goals, strategies and responsibilities incorporating prevention and early intervention strategies to promote optimum outcomes for patients.

  • Ensuring that each consumer's treatment is regularly reviewed comprehensively, including (but not limited to) mental health status, physical health status, treatment goals in order to maintain themselves in their community.


  • To be considered for this position you must be an Australian Citizen or Permanent Resident, or have a relevant visa allowing you to live and work in Australia with no restrictions. Corryong Health does not undertake visa sponsorship for this role.

  • Tertiary qualifications in Psychology, Social Work (accredited mental health social worker), Nursing (credentialed mental health nurse), Occupational Therapy (accredited mental health occupational therapist) or relevant discipline.

  • Current registration with AHPRA as well as the appropriate peak body such as the Australian Psychological Society or Australian Association of Social Workers (AASW) or equivalent.

Registered Nurse and Midwife

Tracey Damm

I’m an ANUM here at Corryong Health and have been working here for over 20 years. My role is very varied and rewarding. Working and living in a small community means we usually do something different every day.
I run a small Day Procedure Unit which we mostly perform Endoscopy with a visiting Surgeon from Albury Wodonga and a visiting Anesthetist from Mt Beauty.
We have a 2 chair Dialysis Unit with 2 permanent patients.
I also run an Antenatal Education program and do the Domiciliary Visits for Albury Wodonga Health when Mum and Babe first come home from hospital.
Rural Nursing is a very rewarding career with a huge variety of clients coming to the facility. Corryong Health is a very important part of the community.
