Requirements
The Home Care Support Worker is responsible for providing a range of services for clients who may have a disability or frail aged persons who have been assessed as needing low or high-level care. To achieve this, the care worker provides assistance in accordance with the identified support requirements as agreed with the client and stated in their care plan.
We have the need for two kinds of Home Care Support Workers:
A Personal Care Worker (PCW) who holds a minimum certificate 3 in HACC, Aged Care or equivalent, or
Personal Care Assistants (PCA) who hold no qualifications but works under supervision.
Responsibilities include:
To assist with personal care and household tasks as outlined in the clients care plan,
To assist with facilitating transport and community access as required,
Household domestic cleaning tasks,
Perform tasks associated with personal assistance, such as assisting with showering, assisting client with shopping, banking, paying accounts, writing letters, assisting with cooking meals, and assisting clients with other duties as requested,
Provide social and emotional support and companionship to the client or the carer of the client
To take action according to emergency procedures,
To support the implementation, adherence and review of an Elder’s care plan/task list
What we are looking for:
Have work rights in Australia,
Certificate III HACC, Aged Care or equivalent/higher (required for PCW),
Previous experience in an aged care (optional),
First Aid and CPR Certificate (not required for PCA),
Basic computer skills,
Victorian / NSW Driver’s licence.

CH is funded by Commonwealth and Victorian State Government funding, and provides a broad range of hospital, residential aged care, medical and primary health and community support services. We provide services to the Corryong community and operate a Medical Clinic, Health and Fitness Centre and Retirement Village.
